OLTC failures is the commonest failure associated with Transformers.
There are different types of OLTC failures. The failure of the motor, a problem with the spring assembly, etc can be reasons for the failure.
If the OLTC does not operate at all there will be issues with the system voltage and the inability of the system to respond to changes in the supply voltage.
However, there can be serious issues such as transformer fire or explosion if the OLTC does not make proper contact with the winding terminals. The arcing thus produced can result in a fire or can cause the vaporization of oil which can result in an explosion.
There are different means of monitoring the OLTC. Vibration monitoring of the OLTC gives a reasonably good idea of its condition.
